MEDICAL TREATMENT AND EMERGENCY FORM
All Marching Band, Indoor Percussion, Fall & Winter Color Guard, Indoor Winds and Show Choir students must have this form on file with the music office.
Updated physicals should be turned in to the school nurse. A current physical must be on file with the school nurse in order to participate. Physicals are valid for 13 months. You may fax them to: 508-961-1620 or email them to the nurse contacts below.

All Marching Band, Indoor Percussion, Fall & Winter Color Guard, Indoor Winds and Show Choir students must complete this form along with completing the online course.

HAZING
Massachusetts Hazing Law
The hazing of any student is prohibited by Dartmouth High School and Massachusetts State Law. Students will sign a policy statement. Any hazing related issues should be reported immediately to the Director of Music.

CORI
All CORI forms must be completed and dropped off IN PERSON with a valid driver's license for verification. For your convenience, you may drop off this form at any of our schools, or you may bring it to 8 Bush Street before 3:45 p.m. Please reference that the form is being completed for the Music Department.
