PLEASE NOTE: Updated work permit procedure
Work permits will be issued Monday - Friday during the hours of 8AM- 3PM excluding holidays
Step 1: Download the Employment Permit Application found at the bottom of this page . Fill out all necessary sections and have the application for a work permit signed by the employer. Note: You must have the permit application signed by the employer , a parent and the student before proceeding to the next step. This form is not the work permit, it is only an application for a work permit.
Step 2: Email the completed permit application and a form of ID to Lauren Pusey at laurenpusey@dartmouthschools.org . In the email, please include the following information and documents :
- Work permit application signed by the employer, parent and student. If the student is under 16, the application must be signed by a physician as well
- Form of ID ( School ID, Copy of Birth Certificate, Drivers License/ Permit , US Passport )
- Students home address
- Students date of birth
- Students city of birth
- Students hair/ eye color
- Students current school and grade . Please note: If the school is other than Dartmouth Public Schools, a letter of good standing will need to be included from the current school
Step 3: The work permit will be filled and electronically signed by an authorized agent from the School Department.
Step 4: The student will receive am email from Adobe Sign , and should follow instructions to electronically sign the form. Once signed they will receive an automated email back with the completed permit in an attachment.
Step 5: Print the work permit and hand to your employer, or email it to your employer if a printer is not available.
Important Notice: Work Permits will only be issued to students who will be working in the state of Massachusetts. If the employer is located in another state, please use that state's official website to view their labor standards/ work permit process.

The state of Massachusetts requires all minors between the ages of 14 through 17 to have a work permit for each employment.
Minors aged between 14 to 15 need to have their physician complete the Physician's Certificate of Health, 16-17 year olds do not need to complete this section.
